Are you looking for additional support and assistance for your child to safely and successfully participate in a Town of Superior recreational program?
The Town of Superior Parks, Recreation and Open Space Department is now partnering with Out & About, a community-based recreation program provider for children and adults with developmental disabilities. This partnership will better assist us in determining when and how to best provide additional resources for program participants who may need them. Families of children with disabilities are asked to complete a Support and Assistance Questionnaire (SAQ) during the course of registration for any Town of Superior program or activity. The SAQ is designed to help us gain a better understanding of your child’s needs and strengths and to help us identify the specific supports and assistance that may be needed for your child to safely and successfully participate in recreational activities. After receiving the completed SAQ, a Parks and Recreation Department staff member will review it and then contact you to help clarify any information on the form, answer any questions you may have and determine the next step in the process.
SAQ forms are available below and at Superior Town Hall. Completed forms must be turned in to Bryan Meyer at the Superior Parks and Recreation Office or emailed to email@example.com at least 2 weeks prior to the program’s start date. SAQ information may be shared with consultants from Out & About but otherwise will remain confidential.
